Laser Hair Removal Medical Director Protocol and Agreement with Electrologists

This includes 3 documents that you can use to establish the relationship between a supervising physician and an electrologist in the state of Florida. Either a physician or electrologist may find these documents useful.
The first is an example (and fillable) form "Florida Supervising Physician Protocol For Electrologists" that outlines and includes the required Florida Protocol. It is adjustable and can be changed if you would like to add or take away.
The second is "Florida Supervising Physician Agreement For Electrologists" which is a sample fillable document that can be used to establish the agreement between the physician and electrologist. 
The third is "Florida State Requirements / Laser Protocol Checklist" which is a reference form that is supplied by Florida on the requirements to set up the protocol and where to send the protocol once it is completed.
